The State of Kansas, through the Kansas Department of Health and Environment invites organizations pursuing collaborative arrangements with one or more Eligible Providers rural health clinics, federally qualified health centers, local health departments, Native American Sovereign Tribe healthcare facilities, certified community behavioral health clinics, licensed substance use disorder facilities, for the purpose of transforming rural healthcare delivery within a region in a sustainable manner to apply for funding through the Regional Partnership Grant Program RPGP is part of the State’s Rural Health Transformation Plan and is funded under the Cooperative Agreement with the federal Rural Health Transformation Program.
The $44 million RPGP grant enables eligible providers to participate in collaborative arrangements that promote preventive health care and address the root causes of disease, enhance rural providers’ efficiency and sustainability, attract and retain a high-skilled health care workforce, spark growth of value-based care models, and foster the use of innovative technologies.
Goal
Enable Eligible Providers to participate in collaborative arrangements that promote preventive health and address root causes of diseases, enhance rural providers’ efficiency and sustainability, attract and retain a high-skilled health care workforce, spark growth of value-based care models, and foster the use of innovative technologies.
Objectives
All grant projects funded under this RFA must align with at least one of the program objectives listed below:
- Realize meaningful reductions in chronic disease rates and avoidable hospitalizations for complications related to chronic disease in rural Kansas.
- Substantially reduce the number of rural Kansas hospitals with negative operating margins.
- Improve provider-to-population ratios for primary care, dental and behavioral health services and ease nursing and allied health shortages in rural Kansas.
- Have 100% of Medicare and Medicaid beneficiaries in rural Kansas in accountable care relationships by 2031.
- Enable rural Kansas providers to meaningfully engage in data sharing and analysis, expanded use of telehealth and remote monitoring, appropriate use of artificial intelligence, and utilization of consumer-facing technologies.

June 2026 to September 2027
Eligibility Criteria
Any Kansas-based organization that has secured a written commitment from at least one Eligible Provider (other than the applicant) to pursue a collaborative arrangement consistent with the goal, objectives, and requirements of the RPGP may apply. Only one application from an organization should be submitted for each collaborative arrangement, regardless of the number of Eligible Providers participating in the arrangement. An organization may submit multiple applications each for a separate collaborative arrangement, and Eligible Providers may participate in multiple collaborative arrangements seeking funding under RPGP. Providers located in non-rural areas can apply for RPGP funds but only if the arrangement directly benefits rural communities.
