For FHPAP, describe how your agency can meet the following specific requirements:
- Be experienced with FHPAP provider expectations or have a clear, credible plan for developing the capacity to meet these expectations.
- Have experience and/or be willing to use HMIS (Homeless Management Information System) as required by MN Housing. For victim service agencies that are unable to use HMIS, provide an alternative plan or system to track program participants and outcomes.
- Be adequately prepared to participate in Grantee and Funder site visits and file reviews. Be able to provide both case management, and the financial services necessary to meet the requirements of the service models.
- Participate in Dakota County Affordable Housing Coalition (the Advisory Committee) and FHPAP workgroup meetings, which assists and advises on FHPAP activities, policies, and outcomes.
Funding Information
Total funding estimated to be available through this RFP is $4,539,674. LHPA amount is secured for year one and estimated for year two. FHPAP is grant funding through MN Housing and will be dependent upon program funding through the State and grant award decisions. Funding estimates are as follows:
- Local Homeless Prevention Aid (LHPA):
- $769,837 (first year confirmed)
- $1,539,674 (two year estimate)
- Family Homeless Prevention and Assistance Program (FHPAP):
- $1.5 million annually
- $3 million (2 years – estimated dependent on funding award from MN Housing)
Grant Period
The contract term for FHPAP services will be from October 1, 2023 through September 30, 2025.

- FHPAP Evaluation and Selection Criteria:
- Be familiar with Dakota housing resources.
- Have clear access point(s) in Dakota County for clients seeking services.
- Experience with FHPAP or have a clear, credible plan for developing the capacity to meet these expectations.
- Have experience and/or be willing to use HMIS (Homeless Management Information System) as required by MN
- Agencies who are victim service providers may have an alternative client and data tracking system to meet reporting expectations.
- Organizational capacity to administer FHPAP funds and services.
- Be able to provide both case management, and the financial services necessary to meet the requirements of the service models.
- Experience proving services to the target population(s).
- Service approach and program model that aligns with community needs and priorities and is an effective service approach for these needs.
- Financial model and budget that meets goals of program
- LHPA Evaluation and Selection Criteria:
- Be familiar with resources, services, and case management for youth in Dakota County
- Have clear access point(s) in Dakota County for clients seeking services.
- Experience with outreach, case management, and providing services to unaccompanied or homeless youth
- Organizational capacity to administer services described, including ability to build relationships with school district staff and partners.
- Be able to provide outreach, case management, and direct financial assistance as proposed.
- Experience proving services to the target population(s).
- Service approach and program model that aligns with community needs and priorities and is an effective service approach for these needs.
- Financial model and budget that meets goals of the program
- Have experience tracking goals, outcomes and reporting to meet reporting expectations
