Applicants are expected to conceive, design, develop, direct, and conduct formative qualitative and mixed method research to explore factors that impact QOL for PWH50+, translate findings into strategies to improve QOL for PWH50+, assess strategies for acceptability with PWH50+ and/or their HIV service providers, and disseminate findings to organizations and partners serving PWH50+. Findings from this NOFO are expected to contribute towards improving QOL for people with HIV and are aligned with the National HIV/AIDS Strategy 2022-2025 (NHAS) and the goals of the national Ending the HIV Epidemic in the U.S. (EHE) initiative, Treat Pillar.

National Goals

Goal 2: Improve HIV-related health outcomes of people with HIV.
- 2.3: Increase retention in care and adherence to HIV treatment to achieve and maintain long-term viral suppression and provide integrative HIV services for HIV-associated comorbidities, coinfections, and complications, including STIs.
- 2.4: Increase the capacity of the public health, health care delivery systems, and health care workforce to effectively identify, diagnose, and provide holistic care and treatment for people with HIV.
Goal 3: Reduce HIV-Related Disparities and Health Inequities.
- 3.4 Address social and structural determinants of health and co-occurring conditions that impede access to HIV services and exacerbate HIV-related disparities.
Goal 4: Achieve integrated, coordinated efforts that address the HIV epidemic among all partners and interested parties.
- 4.2: Increase coordination among and sharing of best practices from HIV programs across all levels of government (federal, state, tribal, local, and territorial) and with public and private health care payers, faith-based and community-based organizations, the private sector, academic partners, and the community.
- 4.4: Foster private-public-community partnerships to identify and scale up best practices and accelerate HIV advances.

Objectives/Outcomes

The objectives for this research are:

Engage PWH50+ and their providers to explore key concerns related to quality of life for persons with HIV, including factors related to physical and mental health and social determinants of health.
Identify acceptable strategies to improve QOL for PWH50+.

The expected outcomes for this research will contribute to our understanding of the issues impacting PWH50+ by:

Increasing knowledge of the factors that impact QOL among PWH50+.
Dissemination of acceptable strategies to intervene on QOL for PWH50+.

Target Population

The target population for this research is persons with HIV who are 50 years of age and older (PWH50+). The sample of participants can be recruited from the community and represent those engaged in defined groups and settings, such as residents of long-term care facilities, patients and staff in HIV clinics or healthcare systems, or persons in a specific geographic area. PWH50+ commonly report social isolation, depression, trauma, polymorbidity, challenges in activities of daily living, and adverse experiences associated with social determinants of health including food insecurity, housing instability, financial insecurity, and unemployment.

Applications should use local area data to identify communities of PWH50+ that may be disproportionately affected by factors associated with poor QOL, and to identify strategies for these communities that have the potential to reduce this disproportionate impact.

Goal 2.5 of NHAS 2022-2025 is to expand capacity to provide whole-person care to older adults with HIV and long-term survivors. It calls for identifying models of care that meet the needs of older PWH, identifying best practices for addressing psychosocial and health needs, and increasing capacity of service providers to support older PWH. Participants should not be excluded from the study based on disability, sexual orientation, limited health literacy, or other social or economic vulnerabilities.
