Building on relevant advancements and lessons learned from the previous funding opportunity (CDC-RFA-DP18-1817), this Innovative Cardiovascular Health Program will build towards the next phase by focusing on partnerships to improve the overall prevalence, detection, and control of hypertension and hypercholesterolemia in priority populations and address SDoH, stress/mental health, health inequity, and injustice – all factors that can lead to and/or exacerbate CVD.
