Details:

The purpose of this program is to ensure the highest possible health status for Urban Indians. Funding will be used to support the 4-in-1 health program objectives. These programs are integral components of the IHS health care delivery system. Funds from this effort will ensure that comprehensive, culturally acceptable personal and public health services are available and accessible to Urban Indians.

Each grantee shall provide health care services under this award only to eligible Urban Indians living within the urban center in which the Urban Indian Organization (UIO) is situated. An “Urban Indian” eligible for services, as codified at 25 U.S.C. 1603(13), (27), and (28), includes any individual who:

Resides in an urban center, which is any community that has a sufficient Urban Indian population with unmet health needs to warrant assistance under the IHCIA, as determined by the Secretary, Health and Human Services (HHS), and who meets one or more of the following criteria:

  • Irrespective of whether he or she lives on or near a reservation, is a member of Tribe, band, or other organized group of Indians, including:
    • those Tribes, bands, or groups terminated since 1940, and
    • those recognized now or in the future by the state in which they reside, or
  • Is a descendant, in the first or second degree, of any such member
  • Is an Eskimo, or Aleut, or other Alaska Native; or
  • Is a California Indian;
  • Is considered by the Secretary of the Department of the Interior to be an Indian for any purpose; or
  • Is determined to be an Indian under regulations pertaining to Urban Indian health that are promulgated by the Secretary, HHS.

Each grantee is responsible for taking reasonable steps to confirm that the individual is eligible for IHS services as an Urban Indian.

Funding Information

  • The total funding identified for fiscal year (FY) 2022 is approximately $8.5 million.
  • Individual award amounts for the first budget year are anticipated to be between $160,000 and $650,000. New applicants may apply for funding up to $200,000; current 4-in-1 grantees may apply for funding up to the amount approved in the last noncompeting award and must demonstrate that they have complied with previous terms and conditions of their award.
  • The period of performance is for 5 years.

Eligibility Criteria

To be eligible for this FY 2022 funding opportunity, an applicant must be an Urban Indian organization, as defined by 25 U.S.C. 1603(29), that is currently administering a contract or receiving a grant pursuant to 25 U.S.C. 1653. The term “Urban Indian organization” means a nonprofit corporate body situated in an urban center, governed by an Urban Indian controlled board of directors, and providing for the maximum participation of all interested Indian groups and individuals, which body is capable of legally cooperating with other public and private entities for the purpose of performing the activities described in 25 U.S.C. 1653(a). Applicants must provide proof of nonprofit status with the application, e.g., 501(c)(3).
