Health First Foundation is seeking  Letters of Intent for its Community Health Grant (NACHG) Program to support high impact programs and projects promoting health and well-being in northern Arizona.

Details:

The focus is on innovative programs addressing the prevention and mitigation of chronic health conditions through education, prevention, diagnosis, or treatment.

In northern Arizona, chronic conditions identified as priorities are diabetes, heart disease, obesity, reparatory conditions, cancer, and dental disease. The Foundation will evaluate opportunities to invest in nonprofit programs and collaborations that demonstrate a positive and sustainable impact on community health.

The Foundation will consider funding nonprofit programs and collaborations that demonstrate a positive and sustainable impact on community health.

Focus: Preventing and mitigating chronic health conditions

  • Chronic conditions:
    • Diabetes, heart disease, obesity, respiratory conditions, cancer and dental disease are significant health priorities in their region
  • Prevention:
    • Education, screening, or early intervention
  • Mitigation:
    • Diagnosis, treatment, or services during recover

Funding Information

  •  Funding is prioritized on projects requesting $50,000 and above
    • Meritorious projects requesting less than $50,000 may be considered

Eligibility Criteria

  • Must be an Arizona 501(c)(3) organization, government agency, school, university, municipality, or tribal entity
    • Religious organizations may apply for funding of non-sectarian initiatives.
    • Organizations without 501(c)(3) status must use a fiscal agent to apply on their behalf
    • Organizations must be in good standing with the IRS at the time of application review
  • Projects must serve populations within northern Arizona
    • Apache County
    • Coconino County
    • Mohave County
    • Navajo County
    • Yavapai County
  •  Funding is prioritized on projects requesting $50,000 and above
    • Meritorious projects requesting less than $50,000 may be considered
  • Project is not a continuation of an existing program or project
  • One application per each qualified organization will be accepted
  • Organization must maintain an active non-discrimination policy
  • Applications using acronyms will not be considered
