The Minority Health Program (MHP) provides grant funding for community-based organizations and Tribal Nations serving economically-disadvantaged minority populations across Wisconsin. The purpose of the MHP Grant Program is to support the projects from organizations serving communities of color, especially those organizations located in areas where health disparities are high to help advance health equity across Wisconsin.

This Request for Applications (RFA) in an effort to support the work of community-based organizations serving economically disadvantaged minority groups and minority group members, especially those organizations that are located in areas where health disparities are high. The 2023-2024 Community Grants will fund projects from community based organizations that are either evidence or science-based, reflects promising practices (practice-based evidence), or which otherwise presents strong rationale of its ability to help eliminate health disparities, achieve health equity, and improve health across the lifespan.

Objectives and Scope

The goals of the work to be funded through Community Grants are to support the work of community-based organizations serving economically disadvantaged minority groups and minority group members, especially those organizations that are located in areas where health disparities are most pronounced. The goals of the work funded help to improve health status and eliminate health disparities among economically disadvantaged minority groups in Wisconsin through the promotion of individual, community, and population health. The five priority areas for activities includes:

  • Addressing social and community conditions, such as: economic well-being, supportive systems of dependent care, healthy housing,
  • Physical, mental, and systemic safety,
  • Person and community-centered health care
  • Social connectedness and belonging
  • Mental and emotional health and being

Funding Information

  • It is the intention of the Department to award multiple contracts up to $50,000 each, for the Community Grants for the services required in this solicitation.
  • The contract shall be effective on July 1, 2023 and shall run through June 30, 2024. The Program estimates that disbursement of funds may begin by August 2023, or shortly thereafter.

Eligibility Information

  • Only private nonprofit corporations can apply.

Only proposals that include all the following elements will be considered for funding. Your proposal must:

  • Incorporate 50% match in the budget plan (funding or in-kind).
  • Clearly indicate which community (or communities) of color will directly benefit from your project. The four key community groups are: Black/African American, American Indian, Hispanic/Latino, and Asian American. (Wisconsin Legislature: 250.20)
  • Specify at least one priority area from Wisconsin’s State Health Improvement Plan, that your project will incorporate or impact. The current five priorities are: Social and community conditions (economic well-being, supportive systems of dependent care, and healthy housing); physical, mental, and systemic safety; person and community- centered health care; social connectedness and belonging; and mental and emotional health and well-being.
  • Include an evaluation plan for the project.
