Eligible programs include
- Health care services:
- Programming that provides primary, diagnostic, or preventative services. This would include medical, dental, mental health, and prescription drug treatments.
- Early detection services:
- Programming that provides screening and evaluation services for early detection or prevention of chronic medical conditions.
- Services for patients with Parkinson’s Disease:
- Programming that provides therapeutic services for patients with Parkinson’s Disease or severe arthritic conditions.
- Hospice:
- Programming that provides hospice or palliative care services to those who would not otherwise have
access.

Preference will be given to:
- Programs that leverage resources and that demonstrate collaboration between two or more organizations
- Programs with proven success or those most likely to achieve the goals of the program with an identified population.
Grant Amount
Grants may be awarded in the range of $5,000 – $40,000. Programs may be new, continuing, or existing. Previous grantees may reapply for additional funding if the program successfully achieved its goals.
Criteria
- Service Area: Oklahoma, Cleveland, and Canadian Counties
- The applicant organization must be a 501(c)(3) nonprofit with a current, updated GiveSmartOKC profile.
- Programs must provide direct health care services in central Oklahoma.
- Organizations must have a governing board located in central Oklahoma.
- Grants will not be made to individuals or private foundations.
