The Prime Health Foundation (PHF) is seeking applications for its Grant Program to support organizations, programs and initiatives that demonstrate potential for improving the health of their community.

Details:

Focus Areas

  • Prevention or early detection of diseases. e.g.,
    • Projects that provide community-based health screenings
    • Projects that provide routine dental exams
    • Projects that make healthy foods and beverages affordable, available, and the desired choice
    • Projects that increase physical activity and healthy exercise habits
  • Improvements in the quality, availability and/or accessibility of health care services. E.g.,
    • Projects that increase the reach and impact of critical physical and behavioral health services (note change in wording)
    • Projects that reduce unnecessary and/or excessive health care costs
    • Projects that provide consumers the information and tools they need to manage their care, and make smart health decisions for themselves and their families
    • Projects that address distance/transportation barriers to health care
  • Improvements in conditions that lead to, or outcomes that are the result of, health care disparities affecting young men of color, ages approximately 14 – 44. E.g.,
    • Note: Projects in this category target the social, economic, and/or environmental factors that shape the health of young men of color, ages approximately 14 – 44. These include factors such as structural racism, employment, education, housing and neighborhood conditions. Requests for funding in this category should support a new project specifically designed to meet the needs of this population, rather than fund an existing project that already works with this population.
      • Projects that offer trauma-informed services to address hopelessness and/or adverse childhood events
      • Projects that aim to improve fathering skills, male development and/or job readiness
      • Projects that remove legal barriers to healthy living
      • Projects that improve health outcomes for young men of color

Funding Priorities

In addition to falling within one of their three Focus Areas, all projects must:

  • Address a critical need in the community
  • Demonstrate culturally responsive practices
  • Target persons who are poor, uninsured, and/or underinsured, and
  • Target persons who live in the seven-county area served by PHF: Jackson, Clay, Platte, Cass, and Ray counties in Missouri and Johnson and Wyandotte in Kansas

Funding Information

The average amount funded to grantees is approximately $16,000.

Criteria

Additional criteria to be used in project evaluation are whether the proposed project/applicant:

  • Demonstrates organizational stability and the existence of staff expertise to execute on the project
  • Offers the potential of sustainable solutions
  • Operates under clearly articulated objectives and with measurable outcomes
  • Collaborates with other community agencies (financial and programmatic)
  • Is locally governed, operated and funded
  • Develops or tests an innovative approach to service delivery
