Eligibility Criteria
To be eligible for this grant, applicants must meet the following criteria:
- Must have a documented diagnosis of female factor infertility from a licensed healthcare provider, affecting the applicant’s ability to conceive or carry a pregnancy.
- Must be a US citizen or permanent resident.
- Must be seeking treatment at an accredited fertility clinic within the United States.
- Must demonstrate financial need and provide details regarding the anticipated costs of treatments or processes related to achieving parenthood.
- Applicants must submit the Grant Medical Form related to physical and mental well-being.
- Applicants must hold basic medical insurance for mother and baby, including prenatal coverage. Applicants are not required to have fertility coverage.
- All applicants must maintain adequate living arrangements.
- Applicants must demonstrate the financial ability to support the child.
- Grant recipients must utilize funds within 12 months of receiving a grant, unless otherwise approved by the Hopeful Mama Foundation Board of Directors.
- Hopeful Mama Foundation does not reimburse for previous treatment.
