The Black Physicians & Healthcare Network (BPHN), a program of the National Center for Children and Families (NCCF), invites community-based organizations to submit bold, innovative proposals to advance health equity for Black residents in Montgomery County, Maryland. This funding opportunity aims to expand culturally relevant health education, direct outreach and intervention efforts that reduce health disparities and improve health literacy.
NCCF seeks to fund dynamic organizations that are committed to:
- Delivering direct engagement and small-large group education to increase health literacy and awareness to influence behaviors and improve health outcomes.
- Creating and disseminating culturally tailored wellness resources, outreach activities, and projects that focus on a variety of priority health areas that negatively impact Black residents in Montgomery County. There will be increased emphasis on proposals who have outreach or project goals that focus on:
- Underrepresented areas in the Upcounty region of Montgomery County (i.e., Poolesville, Damascus, Clarksburg)
- Men’s Health & Wellness
- Maternal & Child Health & Wellness
- Dental Education
- Returning Citizens from Incarceration
- Nutrition & Obesity
- Mental Health (emphasis on children, adolescents, men, perinatal, and suicide prevention)
- Alzheimer’s & Dementia Education
- HIV/AIDS & STI Prevention
- Cancer Prevention & Screening
- Chronic Disease Prevention and Management
- Leading community-driven health interventions such as resource fairs, workshops, and support groups that reduce cultural and socioeconomic barriers.
- Projects must center the voices and experiences of Black communities and demonstrate a measurable impact on health behaviors and outcomes.
Funding Information
Up to $75,000. This is a reimbursement grant.
Grant Period
July 1, 2025 – June 30, 2026.
Eligibility Criteria
Eligible applicants must be:
- A nonprofit, community-based organization serving Black residents in Montgomery County;
- Able to conduct at least 80% of project outreach and activities in person;
- Prepared to engage in quarterly coordination meetings, submit monthly impact reports, complete accurate, detailed invoices in a timely manner, and participate in the BPHN program evaluation.
