The funding aims to assist providers with technology and administrative related costs, business and operational processes, and workforce to ensure successful delivery of HRSN services to qualified OHP members.
These funds support community partners who would like to enroll as Medicaid providers to deliver social needs benefit services in the areas of climate, housing, nutrition, and outreach & engagement.

Uses and Funding Priorities
- CCBF can be used for technology related costs, development of business and operational processes, administrative costs, and outreach and education to OHP members.
- Housing providers that serve boundaries beyond Yamhill County.
- Nutrition Providers.
- Culturally specific and rural outreach and engagement Organizations.
- Organizations that have received CCBF before are eligible to apply, but cannot apply for funding for the same or existing staff position.
