IHN-CCO and community partners, through the Delivery System Transformation Committee (DST), welcome innovative ideas and collaborative strategies to ensure all individuals have equal opportunities to be healthy where they are born, live, learn, work, play, worship, and age.
Through embracing the values of equity, inclusion, innovation, respect, and collaboration, the DST envisions a healthcare system rooted in community knowledge, values, and innovation. The DST is committed to fostering an inclusive, engaged, collaborative space where innovative and transformational ideas are shared and supported with the intent of advancing health equity, centering community perspectives, and improving health outcomes for diverse communities across the Benton, Lincoln, and Linn County region.
Priority Areas
In alignment with the 2024-2028 Community Health Improvement Plan (CHIP), the DST committee has chosen the following priority areas. To be considered for funding, all proposals must address one or more of the areas below and clearly indicate how the proposed project would advance relevant CHIP goals and strategies for the chosen priority area(s). All proposals must also clearly address Inclusion, Diversity, Anti-Racism & Equity (IDARE).

- New Partner Engagement Funds: $15,000
- Pilot Project Funding: Up to $150,000

Eligibility Criteria
- Anyone who can make a positive impact on the health and wellbeing of IHN-CCO members and the broader Benton, Lincoln and Linn County community is welcome to apply. Eligible applicants may include nonprofit 501(c)(3) organizations, schools (including charter schools), public school districts, educational service districts, county public health agencies, federally qualified health centers (FQHCs), companies/LLCs, groups and coalitions, and individuals. Organizations under fiscal sponsorship may also apply. Faith-based organizations may apply, but only if faith-based components are non-compulsory. Applicant must be a new partner to the DST. This means both of the following must be met:
- Applicant has not previously been awarded DST pilot project funding.
- Applicant has not previously been awarded DST New Partner Engagement Funds for more than one year. Awardees may only receive this funding twice.
- For example, an applicant cannot have received funds in both 2024 and 2025 and apply for this funding again in 2026. In this situation, applicants would be considered eligible for pilot project funding.
